I attest that by signing this permission form that I am a legal guardian of (or have been given permission by the guardian of) the child or children that I am signing up for this event/trip.

I hereby release The Center, and its leaders from any liability in the event of accident or injury to my child.

I understand that every effort will be made to contact me in case of emergency, but in the event that you cannot be reached, I authorize the treatment of this child by qualified medical personnel.

I consent to allowing this child's picture to be used by The Center.
